Substance Use Disorder
Individuals with moderate to severe substance use disorders who need support with recovery skills, relapse prevention, and connecting to community resources.
Adults with Serious Mental Illness
Adults with schizophrenia or schizoaffective disorder needing support with medication adherence, daily living, and community integration; individuals with bipolar disorder with mood instability affecting work and relationships; and adults with severe major depressive disorder impairing independent functioning.
Co-occurring Disorders
Individuals with both mental health and substance use disorders who need integrated support managing symptoms, treatment, and daily functioning.
Individuals with Non-SMI but Functional Impairment
Individuals with adjustment disorder after major life events (e.g., divorce, job loss) needing short-term support for coping and daily structure, and those with anxiety disorders impairing their ability to work, attend school, or manage daily responsibilities.
High-risk / complex psychosocial populations
Individuals with significant psychosocial challenges (e.g., homelessness, frequent hospitalizations, justice involvement) who need help with stability, coordination of care, and independent living skills.
